 Acceptable ID Documents

 

Primary ID Documents

  • United States Passport or a U.S. Passport Card
  • Permanent Resident Card or an Alien Registration Receipt Card (Form I-551)
  • Foreign passport
  • Employment Authorization Documents with a photograph (Form I-766)
  • Driver’s License or an ID card with photo - Issued by a U.S. state or territory
  • U.S. Military ID card
  • U.S. Military dependent ID card
  • Personal Identity Verification (PIV) card

 

Secondary ID Documents
The secondary identity source document may be from the list above, but cannot be of the same type as the primary identity source document.

  • U.S. Social Security Card issued by the Social Security Administration
  • Original or certified copy of a birth certificate issued by a state, county, municipal authority, possession, or outlying possession of the U.S bearing an official seal
  • ID card with photo - Issued by a federal, state, or local government agency or entity
  • U.S Coast Guard Merchant Mariner Card
  • U.S. Citizenship Certificate (Form N-560 or N-561)
  • U.S. Citizen ID Card (Form I-197)
  • Identification Card for Use of Resident Citizen in the U.S. (Form I-179)

 CONTRACTORS

 

Contractors Deploying Overseas ID

  • 2 Forms of acceptable ID
  • SOFA agreement
  • SPOT LOA
  • DD Form 1172-2 requesting the overseas ID card 

NOTE: Forms must be properly filled out or they may be rejected.

 DEPENDENTS

 

All Dependents
Your sponsor must be present with you at the time of the appointment, in order to get or renew a Dependent ID, as well as the proper documents. 

If your sponsor cannot be present, you must have a signed DD Form 1172 from the sponsor.  Service members can access a DD Form 1172 online or printed by a MPS technician.  DD Form 1172 are only valid 90 days after printed by a technician.     

 

Power of Attorney
If the military sponsor is unable to accompany you to the visit, a valid General Power of Attorney to allow you to conduct business on the sponsor’s behalf.  You may get this document at the Legal Office.

Marriage

  • Original marriage certificate to update DEERS. 
  • 2 Forms of acceptable ID (including MIL-to-MIL marriages)

Foreign National
If your spouse is a foreign national:

  • Marriage certificate
  • Valid passport
  • Birth certificate

NOTE: All documents must be certified and translated into English.

 

Child Dependent ID

  • MIL-to-MIL dependents can receive an ID as a newborn. 
  • All other dependents are eligible for an ID at the age of 10.  Sponsor must be present.

 

Adding Newborns to DEERS
The sponsor must bring:

  • Original birth certificate
  • Social security card

* Proof of birth from the hospital, is permitted if the sponsor hasn’t received them.

 

Adding Step-children to DEERS
The sponsor and spouse must bring:

  • Original marriage certificate
  • 2 Forms of acceptable ID of the new spouse
  • Child’s or Children’s social security card(s)
  • Child’s or Children’s Birth Certificate(s)

* If originals cannot be provided, ONLY certified copies will be accepted.

 

Full-time College Students

  • College students between the ages of 21-23 must bring:
  • 2 Forms of acceptable ID
  • DD Form 1172
  • Letter from their school registrar’s office, showing proof of current full-time enrollment (12 credits or more).  Letter must be from an accredited institution of higher learning in pursuit of an Associate Degree or higher. 

* Initial college student IDs must be issued on or after the 21st birthday.
** If the student is 23 or older the sponsor must contact Tricare to apply for the Young Adult Program until age 26.   1-877-988-9378.

 

Dependent Ward

  • DFAS letter of approval
  • Court Documents placing the child in the member’s household for 12 months
  • Ward’s Birth Certificate
  • Ward’s Social security card
  • DD Form 1172

Dependent Parent

  • DFAS letter of approval
  • Service member’s Birth Certificate
  • Parent’s Valid Photo ID
  • Parent’s Social Security card
  • DD Form 1172

Dependent Parent-in-Law

  • DFAS letter of approval
  • Spouse’s Birth Certificate
  • Service member and spouse’s marriage certificate (if spouse is not in DEERS)
